Charlotte House in Isleworth is celebrating national recognition once again after Hannah Carrera, Unit Head for Palliative Care, won Best Care Manager at this year’s Palliative and End of Life Care Awards.
The award, which was presented by Kate Garraway, highlights the home’s continued excellence in supporting residents and families through some of the most challenging moments of life. Carrera, who leads Charlotte House’s specialist palliative care unit, was praised for her compassion, professionalism and unwavering dedication to ensuring residents receive thoughtful, person-centred support.
Home Manager Elisabeta Surligeanu said the accolade reflects the values that underpin the home’s work. “We are all so proud that Hannah’s compassion has been recognised on the national stage,” she said. “She's hardworking and utterly dedicated to the residents of Charlotte House, making sure they and their loved ones have a positive, empathetic experience of end-of-life care. Congratulations, Hannah!”
The awards, organised by Care Talk magazine, celebrate outstanding practice across social care, hospices and community hospitals. They are designed to shine a light on the complex and often unseen work carried out by palliative care professionals—work that can involve supporting people living with dementia, neurological conditions, complex physical illnesses and other life-limiting diagnoses.
What makes the event distinctive is its focus on social care-led initiatives: projects and approaches developed by care home teams, community carers and specialist units that improve how people live and die towards the end of their lives. Categories span the breadth of the sector, recognising excellence among services for older people, those with learning disabilities or autism, volunteers, and organisations developing innovative tools and practices.
